                                                             2C Cardiology/Medical Inpatient Unit


The Nurse Practitioner – Cardiovascular and Medicine Program (NP‑CVM) reports to the Program/Service Director

and has a matrix relationship for nursing practice to the Director of Nursing. NP’s will have a professional

accountability for quality and safety to the Medical Advisory Committee through the Chief of Department and matrix

relationship to the Chief Nurse Executive. The NP is an expert practitioner with a specialty based focus who functions

with a high level of autonomy, knowledge and skills to formulate clinical decisions and manage chronic illness

through an interdisciplinary team approach. NP practice addresses the five domains of Advanced Practice Nursing:

Clinical Practice, Collaboration, Research, Leadership, and Change Agent.


The percentage of time dedicated to each of these domains is variable, based on the program needs and will evolve

with the program.

‑ Proactively identifies patients with advanced CHF and COPD

‑ Supports ED physicians and Hospitalists to optimize patient care and outcomes

‑ Identifies Low Risk patients and defers admissions if possible utilizing rapid response clinic options &/or referral

back to their family health team within 48‑72 hours

‑ Collaborates with Care Team to optimizes treatment plan while in hospital to improve patient outcomes and

decrease Length of Stay

‑ Planning and referring to appropriate outpatient programs to optimize patient’s treatment plan



*  Unencumbered registration as a Registered Nurse in the Extended Class with the College of Nurses of Ontario.

*  Nurse Practitioner certification in Primary Health Care or Adult streams.

*  Membership in RNAO or NPAO is required.

*  Successful completion of Graduate Studies.

*  Minimum 3 years recent nurse practitioner experience in an acute care setting during past 5 year period.

*  Experience in cardiac/chronic disease management (2 years minimum)

*  Excellent communication skills.

*  Demonstrated commitment to the principles of Patient and Family Centred Care.

*  Current Basic Cardiac Life Support (BLS) Certification.

*  Regular and sustained attendance.

*   Knowledge of and adherence to the Hospital¡¦s Code of Conduct.

*  Computer skills using Word, Excel and Power Point, Groupwise

*  Certified Respiratory Educator



Salary Range:  $55.460 to 58.860
