                                                                      1 ‑ Full Time ‑ Psychologist

                                                       Complex Centre for Diabetes Care (CCDC)

 

The Centre for Complex Diabetes Care (CCDC) provides evidence‑based interdisciplinary medical, nutritional and

psychosocial services to adults and seniors living with diabetes and other chronic health conditions.  Intensive case

management is provided until a patient is able to self‑manage their conditions with support from primary care or other

services.

 

Reporting to the Manager, Centre for Complex Diabetes Care, the Psychologist works within an interdisciplinary

team, collaborating with physicians, nurse practitioners, nurses, dietitians, social workers, physical therapists, and

occupational therapists. Within the team, the psychologist offers conceptualization of patients' presenting concerns

through a biopsychosocial model to enhance the quality of patient care and to improve clinical outcomes. The

Psychologist is responsible for the provision of psychological services including assessment, diagnosis, individual

and group treatment, and team consultation. The psychologist applies an evidence‑based approach to support

enhancement and maintenance of patients' physical, intellectual, emotional, social and interpersonal functioning. The

Psychologist liaises and consults with relevant community agencies and service providers as needed to support

patient care. The psychologist may also provide leadership in clinic processes and clinical program development and

evaluation.

 

EMPLOYMENT REQUIREMENTS:

 

Education/Experience:

* Doctorate from a CPA/APA Accredited Clinical Psychology Program

* CPA/APA accredited internship/residency (or equivalent) in Clinical Psychology

* Registered or eligible for registration as a Psychologist with the College of Psychologists of Ontario is mandatory

* Authorized area of practice in Clinical Psychology is mandatory. Clinical and Health Psychology is preferred

* Authorized client population in adults is mandatory; Authorized client population in seniors is an asset

* Education, training and experience in evidence‑based psychological approaches (e.g., motivational interviewing) for

behaviour change and treatments for a range of common medical and mental health conditions, in individual and

group treatment delivery

 

Assets:

* Education, training and experience in behavior change and common mental health presentations, including eating

disorders, substance misuse, trauma, anxiety, and depression

* Training and experience in the treatment of diabetes and other chronic health conditions

* Experience in providing consultation to interdisciplinary teams in medicine/health psychology and mental health

 

 

Salary Range:  $59.700 to 77.430
